Most badly fitting harnesses start the same way. Someone reads "medium" on a chart, picks the size off the weight from the vet scales, and finds the straps riding halfway up the ribs. Weight says very little about the shape of a chest. A stocky ten kilo dog and a lean ten kilo dog are built completely differently. Take a soft tape, run it around the widest part of the ribcage just behind the front legs, and note that number down. Measure the neck opening as well. Broad-chested, short-muzzled dogs carry their weight forward and low. Standard straps creep up into the throat on that build, which is the exact thing a harness should prevent.
